About this cause
Paul Sartori Hospice At Home helps people in the later stages of a life-limiting illness live with comfort and dignity in their home environment in Pembrokeshire. The organisation not only supports patients, but family members and carers. This project will contribute towards delivering home nursing care, addressing the physical, psychological, social and spiritual needs of patients, their families and carers. Through this project, the End of Life Nursing Care Team, will be able to provide both day respite, giving the family an opportunity to do some shopping, attend appointments or benefit from time away, and night care, which allows families much-needed rest. Both can be accessed, often at short notice, to expedite discharge, or to prevent admission to hospital.
How this cause brings people in the community together
The project will bring families back together by providing quality end-of-life home nursing care. This specialised support means that patients can remain in the comfort of their own homes, surrounded by their family. One family member said: “They treated my father with respect and dignity at all times. Their kindness and professionalism was first class. It was invaluable hospice at home care.”
