We need materials to work with schools to educate and inspire students to understand hospice care and the services available.
About this cause
Our vision is to have a community that holds no boundaries in treasuring lives through the provision of specialist supportive, palliative and end of life care. Our new care model focusses on the entire patient pathway, from diagnosis to end of life and beyond. Our vision is to provide personalised and seamless, high quality palliative and end of life care for our patients and their families in the place of their choice. Creating a relationship with our patients and their families in the last few days of life, whilst important to us, is no longer enough for us. We want to be by someone’s side from the day they are diagnosed with a life limiting illness. We want to be the first place a GP, District Nurse or other referrer wants their patient to be.
How this cause brings people in the community together
Firstly it will educate staff and students on the benefits of a Hospice and the extensive resources available to whole family. Following this, it will create a referral pathway for schools to refer into the hospice for family support/bereavement support. Educating young people has a whole community knock on effect as the message is spread further and lasts longer when captured at an early age.
